Service Request Form

Teacher and Young Student

Please complete this form if your district is interested in contracting with Braxy for speech therapy/related services in Illinois. We know how important it is to have reliable, high-quality support for your students, and we prioritize placing in-person therapists and staff whenever possible.

Once we receive your form, we’ll reach out to match you with a provider who fits your needs based on availability, experience, and location.

At this time, we mostly contract for SLPs and paraprofessionals. However, if your district is in need of other related service professionals (e.g., OTs, PTs), please select "other" and write the professional type in the bottom box. We will try to find a match!

We understand that staffing needs can change quickly, so feel free to email us at schoolcontracts@braxyspeechtherapy.com with updates or questions at any time. We’re excited for the opportunity to partner with you and to ensure your students receive the services they deserve.

Professionals Needed: Required

At Braxy, we deeply value and prioritize in-person therapy services. We believe that having consistent, qualified professionals on-site can make a powerful difference for students, teams, and school culture. Whenever possible, we focus on placing therapists who can be physically present and integrated into your school community.

That said, we also understand the staffing challenges many districts face. If a position remains unfilled, virtual therapy can be an effective and appropriate solution—especially when the alternative is for students to go without services altogether. Teletherapy is evidence-based, proven to be effective across a wide range of student needs, and allows districts to remain compliant with IEP requirements. We’ve supported districts through this process and have seen great success when it’s done thoughtfully.

We’re happy to talk you through the logistics of virtual implementation, including equipment, scheduling, and staff coordination.

In-person is always our first priority—but when needed, virtual can be a strong, legally sound, and student-centered alternative.

Given the above information, would your district be open to virtual services if an in-person provider could not be found? Required

Thank you - we will be in contact soon!
